Inspector Notes

An unannounced, on-site, monitoring inspection was initiated and completed on 09/04/2024, as a part of the licensure period. The on-site inspection began at 1:00 PM and ended at 2:35 PM. The inspector reviewed compliance in the areas listed above. There were 34 children present at nap time and 5 staff. The inspector reviewed 4 children?s records and 4 staff records on site. This inspection included:
- document review (i.e. Injury logs, emergency drill logs, medication administration logs, authorization forms),
- tour of the facility,
- interviews,
- observations, and
- results of lead testing of the water supply.

Information gathered during the inspection determined a non-compliance with applicable standards or law, and a violation was documented on the violation notice issued to the program.

Violations

Standard 8VAC20-780-270-A
Based upon observation, the facility has not ensured that areas and equipment of the center, inside and outside, are maintained to be clean and safe.
Evidence:
1. There is a six inch long open crack in the molded plastic slide that could pinch and injure a child.
2. The metal dividers between the toilet stalls in the boys bathroom are rusted along the bottom and the taping used to cover the rust is peeling off, thus the dividers cannot be adequately sanitized.
Plan of Correction: The facility responded with the following:
1. The slide has been temporarily repaired to be safe while we are searching for a possible replacement slide.
2. The tape between the stalls will be removed, the rusted areas will be sanded and new tape will be placed over the rusted areas.
