During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 01/07/2025. The inspection began at 12:35 PM and ended at 2:25 PM. The inspector reviewed compliance in the areas listed above. There were 31 children present and 6 staff. The inspector reviewed 4 children?s records and 4 staff records. This inspection included:

? observations.

Information gathered during the inspection determined a non-compliance with applicable standards or law, and a violation is documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
At least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members shall obtain and submit the results of a follow-up tuberculosis screening.
the most recent tuberculosis screening in the record of staff 2 was two years and 6 days ago.
Plan of Correction: The facility responded with the following:
Staff 2 was given the required documents to complete a repeat tuberculosis screening. The tracking spreadsheet had not been reviewed yet following winter break.
