Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 08/19/2025, as a part of the licensure period. The on-site inspection began at 10:45 AM and ended at 11:55 AM. The inspector reviewed compliance in the areas listed above. There were 33 children present and 5 staff. The inspector reviewed 5 children?s records and 4 staff records on-site.
This inspection included:

Violations

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition.
There is a four inch crack in the blue spiral slide creating a potential pinch point or could cut a child on the sharp edges. Wear and tear from use has thinned the slide surface.
Plan of Correction: Facility response:
All staff will be immediately notified that the climbing structure is not to be used by the children until further notice.
Maintenance was contacted during the inspection to determine and begin repair options.
