Inspector Notes

An unannounced , on-site monitoring inspection was initiated on 08/18/2025 and completed on 08/18/2025. The on-site inspection began at 9:45 am and ended at 11:50 am. The inspector reviewed compliance in the areas listed above. There were 23 children present and 5 staff. The inspector reviewed 6 children's records and 3 staff records on-site on 08/18/2025.

Violations

Standard 8VAC20-780-510-P
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of.
One prescription medication in the facility had an authorization that expired more than 6 months ago. This medication had been administered to child 1 after the authorization had expired.
Plan of Correction: The parent was contacted during the inspection and asked to update the authorization or pick up the medication by closing.
