Inspector Notes

An unannounced, on-site monitoring inspection was completed on 02/18/2025, as a part of the licensure period. The on-site inspection began at 12:00 pm and ended at 2:25 pm. The inspector reviewed compliance in the areas listed above. There were 45 children present and 9 staff. The inspector reviewed 5 children?s records and 6 staff records. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to obtain documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children.

Staff #2 is currently working and has a TB screening that was dated 10 days after their first day of employment. Staff #3 is currently working and has a TB screening that was dated 11 days after their first day of employment. Staff #4 is currently working and has a TB screening that was dated 2 days after their first day of employment. Staff #5 is currently working and has a TB screening that was dated 14 days after their first day of employment.

Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.

There was less than 2" of resilient surfacing in the fall zones of the playground equipment.
