Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was conducted on 04/14/2021 and 04/15/2021. The Program Director was contacted by telephone to initiate the inspection. There were 51 children present with 6 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance with subsidy regulations, and a violation is documented on the violation notice issued to the facility.

Based on record review, the vendor failed to ensure that documentation of the TB screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment.

Evidence:
1. The TB screening for staff 1 was completed on 03/15/2021 which was after her hire date of 02/18/2021.
2. The TB screening for staff 2 was completed on 06/10/2021 which was after her hire date of 06/09/2020.
Plan of Correction: Going forward all staff will obtain a completed TB screening prior to hire.
