Inspector Notes
An unannounced inspection was initiated on 07/22/2024 and completed on 07/22/2024 in response to a complaint received by the licensing office on 06/07/2024 relating to allegations of insufficient
program and activities, supervision and injury documentation. The inspector reviewed 2 staff records on-site. This inspection included:
*Document review ( injury & incident logs, written policies and procedures);
*Tour of the facility;
*Observations;
*Staff interviews
The preponderance of evidence gathered during the investigation does not support the allegations of inadequate supervision and inappropriate program and activities therefore these areas of the complaint are found to be not valid. The evidence does support the allegation of insufficient injury documentation and this part of the complaint is found to be valid.
Though not a part of the original complaint, a violation that was not reported was found related to diapering procedures.
These violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/01/2024. A POC submitted after this date will not appear on the public website.
Violations
2Evidence:
1. The inspector observed multiple items from lunch, including but limited to, dishes, silverware, cups and food placed on the diaper changing table in the two-year old classroom.
a. Staff 1, who was caring for children in the two-year old classroom, stated there were children in care in diapers and that the diaper changing table was used daily for diapering.
Evidence:
1. Written information reviewed indicated three minor injuries that occurred on 03/28/2024 and 05/20/2024 were not documented in the center injury log.
2. Administrative staff confirmed that written injury reports could not be located for these injuries.
