Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 08/19/2025, as a part of the licensure period. The on-site inspection began at 12:35 PM and ended at 1:45 PM. The inspector reviewed compliance in the areas listed above. There were 32 children present and 4 staff present at nap time. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included:

? observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
2The most recent sanitation inspection by the health department was dated 12/12/2023.
The health department will be contacted today to schedule a sanitation inspection. Note: the food service inspection was up-to-date.
On the back playground there was a 2 foot long 1in by 1in board with 2 nails sticking out from the end.
There was poison ivy growing up a tree on the back playground.
The board was immediately removed from the playground.
The poison ivy was removed from the tree and weed killer will be applied to prevent future growth.
All staff will be reminded to perform routine checks of the playgrounds for potential hazards.
