Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was conducted on 11/10/2021 in response to a self report made to the Department on 10/28/2021. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on interviews, a record review and observation, the center did not ensure all requirements were met where resilient surfacing was used.

There was 4 inches of resilient surfacing in the fall zone of the monkey bars. Six inches is required.
