The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Pursuant to subsection C of ? 22.1-271.2 of the Code of Virginia, documentation of immunizations is not required for any child whose: 1. Parent submits an affidavit to the center on the current form approved by the Virginia Department of Health stating that the administration of immunizing agents conflicts with the parent's or child's religious tenets or practices. Child #1 and Child #2 are both religiously exempt from immunizations and did not have an affidavit notarized in their file.
Plan of Correction: The center responded with the following: Administration will ensure that both Child #1 and Child #2 have original copies of affidavits notarized that they are religiously exempt from immunizations. Moving forward, administration will ensure all appropriate documentation for immunizations are in the records prior to the child?s first day of attendance.
