Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A subsidy health and safety inspection was initiated on May 3, 2021 and concluded on May 4, 2021. The provider was contacted by telephone to initiate the inspection. There were 91 children and 19 staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Based on record review, in one of five staff records reviewed, the vendor did not ensure that staff who work directly with children shall complete the department's health and safety update course.

Evidence: The director confirmed that there was no documentation that Staff #3 (date of hire 7/24/18) completed the Preservice Update training.
Plan of Correction: The training is in progress for the staff person and should be completed this week.
