Inspector Notes

A monitoring inspection was initiated and concluded on 5/20/2022. At the time that the inspector arrived there were 90 children present, ranging in ages from 2 to 5 years old, with 13 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of records and interview, the licensee did not obtain a copy of the results of a criminal history record information check and a search of the child abuse and neglect registry or equivalent registry from any state in which any staff has resided in the preceding five years.
Evidence:
Five staff Records were reviewed.
The record for staff #4 who began employment at the center 5/11/2021 contained a signed Sworn disclosure or affirmation statement which indicated that staff #4 lived in North Carolina within 5 years of employment.
The record for staff #4 did not contain the results of a criminal history record information check and a search of the child abuse and neglect registry or equivalent registry from the state of North Carolina.
The program director confirmed that this documentation had not been obtained for staff #4.
Plan of Correction: The program director will complete the requests for the Criminal Records check and abuse and Neglect Search for the State of north Carolina within 10 days and send a copy of the request and payment (if applicable) to the licensing inspector. The results will be filed in staff #4's record upon receipt.
