Inspector Notes

A monitoring inspection was initiated on 7/25/2023 and concluded on 7/25/2023.There were 82 children present with 14 staff supervising and 2 administrators. The children were observed on the playground, during art time, playing in the gym, and engaged in a variety of centers. A total of 7 child records and 5 staff records were reviewed. The inspection started at 9:20 am and concluded at 11:40 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and staff interview the center did not ensure to obtain a copy of the results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence: Staff confirmed that Staff #2?s first date of employment was 2/6/2023 and they had not obtained an out of state child abuse and neglect record check for Pennsylvania where they had lived within the last five years.
Plan of Correction: 7/26 DCA submitted request for approval. Approval granted 7/27 and was submitted to PA on 7/27/23

Based on record review and staff interview the center did not ensure staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.

Evidence: Staff confirmed that staff #2 (start date 2/6/2023) completed orientation on 3/1/2023, more than seven days of assuming job responsibilities.
Plan of Correction: Director was out 3/1/23 on medical leave at time of employment. 7/27 Another staff member trained to complete orientation.
