Inspector Notes
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on July 20, 2023 regarding a concern related to supervision, physical plant, and special care provisions and emergencies. The inspection was initiated on 7/25/2023 and completed on 9/5/2023. On 8/7/2023, the inspector was on site between 10:40 am to 12:15 pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation does support the allegation; therefore, the complaint is determined to be valid. There were violations cited during the inspection regarding administration, supervision, and physical plant. See the violation notice for more details.
Violations
3Evidence: It was confirmed by staff that on December 2, 20222, in the cafeteria/gymnasium, three wooden folding tables were leaned against a cart of stacked chairs where they are usually chained to the cart. The three tables that were leaned against the cart fell on a child (age 3) in care.
Evidence: On December 2, 2022, Child #1 (age 3) crawled around a cart of stacked chairs to retrieve a ball. The cart of chairs moved and three wooden folding tables that were leaned against the cart fell on Child #1. Staff confirmed that three tables were on top of Child #1. It was confirmed that Child #1 had to obtain outside medical treatment as a result of the incident.
Evidence: On December 2, 2022 Child #1 (age 3) sustained an injury which required outside medical attention. The incident was not reported to the department.
