The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #1, who had been attending for two weeks, did not have documentation of an immunization record on file.
Plan of Correction: Parent will be told the immunization is required today.
Standard 8VAC20-780-150-B
Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department. The immunization records for child #2 and child #3 were not signed.
Plan of Correction: Parents will be contacted and told a signed copy of the immunization must be provided.
Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children. The screening for Staff #5 was completed two weeks after hire.
Plan of Correction: In the future, staff will have the TB screening completed prior to beginning employment and within 30 days.
