Inspector Notes
An unannounced monitoring inspection was conducted in the center from 11:20 AM - 1:25 PM on June 26, 2023. At the time of entrance, 46 children were in care with 8 staff members present. The sample size consisted of 5 children?s records and 5 new staff records. Children were observed during lunch, preparing for and during rest period. . Repeat violations were found and are identified within this report. Areas of non-compliance are identified on the violation notice.
Violations
6Evidence: Staff # 1?s date of hire 5-27-2022 did not have results of a CPS check by the end of the 30th day of hire. The CPS check was dated 7-14-2022 and Center Director confirmed there was not documentation available for review related to obtaining the CPS check for staff #1 within the required timeframe.
Evidence: Staff #1 date of hire was 5-27-2022 and the TB screening was dated 1-20-2022. Center Director confirmed there was not documentation of a TB screening conducted within the required timeframe.
Evidence: Center Director confirmed the center does not have written procedures for abusive head trauma, safe sleep and coping with crying babies.
Evidence: On the diaper changing table in the infant room a single infant toy and a bucket of toys were observed on the diaper changing table.
Evidence: During lunch in the orange room (children?s age range 3-4) one child was observed requesting a second serving of the cheese quesadilla and Staff #7 said ?we don?t have anymore?. Then 4 additional children requested a second serving and then under the direction of the Center Director additional quesadilla were obtained and all 5 children were then given 1/ 2 of the serving size of the cheese quesadilla. Staff #7 confirmed they were not aware they were supposed to give a second serving of items, that meet the USDA requirements when requested and Center owner confirmed they were not aware that the second serving shall meet the USDA serving size.
Evidence: The records for Staff #3, designated as a program lead, does not have documentation in their record to demonstrate they have the training/education required for program lead. Center Director confirmed the missing information.
