Inspector Notes
An unannounced monitoring inspection was started in the center on April 10, 2024 from 2:20 PM until 4:25 PM and concluded on April 17, 2024. At the time of entrance, 33 children were in care with 8 staff members. The sample size consisted of 5 children?s records and 5 staff records. Children were observed playing outside, during story time and singing and dancing.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

4Evidence: Center Director confirmed the most recent copy of the health inspection available for review was dated 1-6-2023.
Evidence:
The following equipment was observed and determined to inadequately meet safety standards.
1.The infant playground had 2 plastic toys that were broken, one with plastic sharp edges and the other with a broken wheel and a protruding bolt.
2. The infant playground had a pink metal helicopter with areas of rust and peeling paint.
3. In the Blue room there was a children?s table with broken and peeling laminate with sharp edges.
4. In the Pink room one wooden bookshelf and the classroom doors have areas of pink peeling paint.
5. In the Orange room there is a broken floor tile that poses a tripping hazard.
6. On the preschool playground there was white bench with peeling paint.
Evidence: Staff #1 was the only staff on the infant playground supervising 3 infant children and there was not documentation indicting they meet the qualifications for program leader.
Evidence: The following groups of children did not have adequate attendance sheets;
1.Staff #1 did not have an attendance sheet for the 3 children they were supervising on the playground
2.Staff #3 did not have an attendance sheet for the 5 children they were supervising in the Blue Room
3. The attendance sheet in the Orange Room had 16 children listed and only 8 were in the classroom.
4. The attendance sheet in the Infant Room had 6 children listed and only 3 were in the classroom.
