Inspector Notes

A monitoring/renewal inspection was initiated on March 14, 2023 and concluded on March 16, 2023 There were 54 children present, ranging in ages from 5 months to age 5, with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
Based on record review and staff interview the licensee did not ensure to obtain a copy of the results of a criminal history record information check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence: Center Director confirmed that they had not obtained an out of state criminal history check or an out of state child abuse and neglect record check for South Carolina where staff #4 (hire date 2-1-2021) had lived within the last five years.
Plan of Correction: Contact SC DSS and criminal records division to obtain an out of state criminal and out of state child abuse and neglect record
Standard 8VAC20-780-260-B
Based on documentation review and staff interviews, the licensee did not ensure to obtain an annual approval from the health department.

Evidence: Center Director confirmed the most recent copy of the health inspection available for review was date 7-16-2021.
Plan of Correction: Contact health department to schedule a health inspection for the center.

Based on document review and staff interview the licensee did not ensure injury prevention procedures shall be updated at least annually based on documentation of injuries and a review of the activities and services.

Evidence: Center Director confirmed they did not have an annual updated injury prevention plan.
Plan of Correction: Complete annual injury prevention plan by using yearly summary of incident/accident injuries for center.
Standard 8VAC20-780-40-K
Based on review of center documentation and staff interviews, the licensee did not ensure the center shall develop written procedures for abusive head trauma.

Evidence: Center Director confirmed there was not a written abusive head trauma policy available for review.
Plan of Correction: Contact our health and safety department for guidance to ensure a policy is made available to add to our injury/illness prevention plan.

Based on record review, 2 out of 5 children's records did not contain all required information.

Evidence: The Center Director confirmed the records listed below are missing the following information;
1.The record for child #1 did not include one parent?s employment location and phone number.
2.The records for children #1 through #5 did not include previous childcare information.
Plan of Correction: Communicate to parents missing information and have them complete noted items to ensure their child's file is complete.
