Inspector Notes

An unannounced monitoring inspection was conducted on 10/18/2023. There were 10 children, ages 2 years -5 years, 3 staff and 2 administrative staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. Medications were observed during the inspection. A total of 3 children?s records and 3 staff records were reviewed. The children were observed during center time. The inspector arrived for the inspection at 10:30am and departed the center at 12:32am.

Discussion on standards related to background checks.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard 8VAC20-780-90-A
Based on observation and discussion with staff, the center failed to ensure that written agreement between the parent and the center shall be in each child?s record by the first day of attendance.

Evidence: Child #1 start date was 9/6/2023 and on the day of the inspection did not have a signed written agreement that contained the following: authorization of emergency medical care should an emergency occur; signed statement that the parent will inform the center within 24 hours or the next business day after his child or member of immediate family has developed any reportable communicable disease.
Plan of Correction: Provider will obtain written agreement from parent.
