Inspector Notes
An unannounced monitoring inspection was initiated on 9/19/2024 and completed on 10/2/2024 in response to a self-report received by the licensing office on 9/13/2024 relating to behavioral guidance. This inspection included - document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Behavioral guidance shall be constructive in nature and age and stage appropriate. Staff #2 placed scotch tape around both the thumbs of Child #1, 3 years old, in efforts to correct thumb sucking.
Plan of Correction: Provider will train staff on appropriate ways to redirect children.
