Inspector Notes
An unannounced renewal inspection was conducted on 12/05/2023 and concluded with an electronic record and document review on 12/13/2023. There were 20 children, ages 3 years - 4 years, and 4 staff members providing direct care and supervision present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies, and nutrition. The children were observed during arrival, self-directed activities, mealtime, and outdoor time. The inspector discussed the following with the provider: hazardous substances. A total of 5 children?s records, 4 staff records and 6 agent/applicant records were reviewed. The inspector arrived for the inspection at approximately 9:00 am and departed at approximately 10:30 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

3Evidence: Staff 1 had a documented hire date of 10/12/2022. The employee eligibility letter was dated 10/13/2022.

Evidence:
1.A table in Class 1 was observed with a broken table leg. Children were observed using the table to play with manipulatives.
2.A part of the kitchen play set in Class 1 was observed with the left side and left door separating from the center, exposing wooden dowels, creating pinch points.
