An unannounced complaint inspection was initiated and completed on 5/29/2025 in response to a complaint received by the licensing office on 5/28/25 relating to care and supervision of a child. One child's record was reviewed. The inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The following acts are forbidden: physical punishment, striking a child, roughly handling or shaking a child, restricting movement through binding or tying, forcing a child to assume an uncomfortable position, or exercise as punishment.
On 5/27/2025, staff #1 picked up child #1 by his arm from a seated position, and pulled him to the table and forcefully sat him down.
