Inspector Notes

An unannounced monitoring inspection was conducted on 5/30/2023. There were 24 children, ages 3 years-5 years and 3 staff and 1 administrative staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. Medication was observed onsite. A total of 3 children?s and 3 staff records were reviewed. The children were observed during active time on the playground. The inspector arrived for the inspection at 10:30am and departed the center at 11:51am.

Discussion on standards related to play areas.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on the review of three staff?s records, the center failed to ensure that a follow up tuberculosis screening for staff members was obtained.

Evidence: Staff #1 most recent tuberculosis screening was dated 11/27/2020 and on the day of the inspection a follow-up tuberculosis screening was not documented in 2022.
Plan of Correction: Provider will ensure that a follow up tuberculosis screening is completed.

Based on observation, the center failed to ensure that in areas used by children of preschool age or younger electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.

Evidence: Licensing Inspector observed 1 outlet along the wall next to the door on the right side of the 3-5 year old roomin the Hill Building that was missing the top outlet cover. There were no children observed in the room but it was accessible to children.
Plan of Correction: Provider installed an outlet cover in the top outlet.
