Inspector Notes

An unannounced renewal inspection was conducted on 12/11/2023. There were 14 children, ages 3 years-5 years and 3 staff and 1 administrative staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. Medication was observed onsite. A total of 3 children?s, 3 staff records and 3 board/agent backgrounds were reviewed. The children were observed during naptime. The inspector arrived for the inspection at 12:20pm and departed the center at 2:07pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on the review of three board member records, the center failed to ensure that all background checks were complete.

Evidence: Board member #1 start date was 08/01/2020 and on the day of the inspection did not have a sworn statement of affirmation documented.
Plan of Correction: Provider will ensure a sworn statement of affirmation is obtained.
