Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 11/14/2024 and completed on 11/14/2024, as a part of the licensure period. The on-site inspection began at 11:12AM and ended at 1:25PM. The inspector reviewed compliance in the areas listed above. There were 19 children present and 4 staff. The inspector reviewed 3 children?s records onsite 11/14/2024 and 3 staff records on-site 11/14/2024.
This inspection included-document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility and observations (this would include observations of photos, recordings, etc.)
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
