Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 5/19/2025 and completed on 5/19/2025, as a part of the licensure period. The on-site inspection began at 11:30AM and ended at 12:51PM. The inspector reviewed compliance in the areas listed above. There were 22 children present and 3 staff. The inspector reviewed 3 children?s records on site on 5/19/2025 and 2 staff records on site 5/19/2025. This inspection included - document review, tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to have employees undergo a background check every five years. The most recent sworn statement for Staff #2 was overdue by a year and one month.

In areas used by preschool children or younger, electrical outlets shall have protective covers. Two outlets did not have outlet covers in the preschool classroom in the Boxwoods building.
