Inspector Notes

Licensing Inspector conducted an unannounced Monitor Subsidy Inspection on May 18, 2023. There were 58 children ages 2 months to 5 years old present under the direct care of 12 staff. The Licensing Inspector reviewed 5 children?s records and 5 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on inspection, the Vendor failed to ensure each building that was built before 2015 and houses a child day programs is equipped with at least one carbon monoxide detector.

EVIDENCE:
Center was not equipped with a carbon monoxide detector.
Plan of Correction: Center will purchase and install carbon monoxide detector for each building.

Based on inspection & discussion with the Center Director, the Vendor failed to develop and implement a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members.
