Inspector Notes

An unannounced, on-site code compliance inspection was initiated on 08/15/2025 and completed on 08/15/2025. The on-site inspection began at 9:45 am and ended at 12:40 pm. The inspector reviewed compliance in the areas listed above. There were 74 children present with 20 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined that non-compliance with applicable code sections, and violations are documented on the violation notice issued to the program.

REPEAT VIOLATION

Each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent them from caring for children under their supervision.

The health exam for Staff 2 was 2 months overdue. The health exam for Staff 3 was 2 months overdue. There was not a record of a health exam on file for Staff 4.

The center is required to obtain a completed sworn statement prior to the employee?s first day of employment.

The sworn statement for Staff 2 was dated 24 days after their first day of employment. The sworn statement for Staff 4 was dated 3 days after their first day of employment. The sworn statement for Staff 5 was dated 1 day after their first day of employment.
Plan of Correction: Moving forward, the sworn statement will be a part of the employment application.

REPEAT VIOLATION

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

The record for Staff 4, employed for one week, did not have documentation of requesting the child abuse and neglect registry check, criminal history check and a sex offender registry check from all states in which the individual has resided in the preceding five years.
