Inspector Notes

An unannounced monitoring inspection was conducted on 10/24/2022. There were 11 children, ages 3 years-5years and 4 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. Medication for children was observed during the inspection. A total of 3 children?s records, 5 staff records and 1 medication were reviewed. The children were observed during handwashing after breakfast and playground time. The inspector arrived for the inspection at 9:22am and departed the center at 11:26am.

Discussion on standards related to play areas. Discussion on standards related to clean, safe and operable.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on review of five staff?s records, the center failed to ensure that an employee was not employed until the center had the person?s out of state criminal history results.

Evidence:
1. Staff #3 was hired on 7/22/2019 and on the day of the inspection did not have the required out of state criminal history check completed.
Plan of Correction: Provider will obtain the required out of state criminal history check.
