Inspector Notes

A monitoring inspection was conducted on 6/09/2023 with the center management. There were 38 children present, ranging in ages from 1 year to 5 years, with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 2 staff records were reviewed. The children were playing outdoors, eating lunch, and preparing for rest.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-240-A
Based on review of two staff files and staff interview, the facility failed to have staff complete the Virginia Department of Education-sponsored orientation course (VA Pre-service) within 90 days of employment. Evidence: Staff 1 (date of employment 5/31/2022) did not complete the training until 9/7/2022 and Staff 2 (date of employment 5/31/2022) did not have documentation of having completed the VA Dept of Education sponsored orientation course.
Plan of Correction: All staff will be expected to complete the VA Pre-service training within 90 days of employment.
Standard 8VAC20-780-280-B
Based on observation, the facility failed to keep hazardous substances in a locked place that prevents access by children. Evidence: In the preschool classroom, the cleaning solution and
sanitizing solution were in an unlocked upper cabinet.
Plan of Correction: The center owner locked the hazardous substances. Staff will be reminded to lock cleaners when not in use.
Standard 8VAC20-780-510-L
Based on observation, the facility failed to ensure that medication is kept in a locked place using a safe locking method that prevents access to children. Evidence: The medication for Child A was on an upper shelf, unlocked in the infant room.
Plan of Correction: All medications will be locked.
Standard 8VAC20-780-510-P
Based on review of medications, the facility failed to notify parents that that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed. Evidence: The medication authorization for one medication labeled for Child A expired in 3/2023. The authorization had not been renewed and the medication was not returned or destroyed as required. The medication was on site on 6/9/2023.
Plan of Correction: The medication will be returned to the parent.
