Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/19/2024 and completed on 9/19/2024. The on-site inspection began at 11:18 A.M. and ended at 4:00 P.M. The inspector reviewed compliance in the areas listed above. There were 38 children present and nine staff. The inspector reviewed five children?s records and five staff records on-site on 9/19/2024. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/27/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, rusting of any equipment.

In the main hallway, a rusting step stool was being used by the children to access the water fountain.

In six classrooms, there was an area rug on the floor that was stained with spots and dirt. Children were observed sitting and playing on the rugs.
Plan of Correction: Step stool was removed and replaced immediately on-site during inspection.
Rug will be shampooed and cleaned

Documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position.

The records for Staff #1 and Staff #2 , did not have documentation of their education in the files. Staff #1 has been employed for 1 year and Staff # 2 has been employed for 5 years.
Plan of Correction: Staff will provide documentation of education as soon as possible
