Inspector Notes
An in person monitoring inspection was conducted today.
All rooms were in ratio during the inspection.
Medications were reviewed and were in compliance. 3 staff and 3 child records were reviewed. See Violation Notice for non-compliances.

2 of 2 staff records reviewed did not have new fingerprint results on file. Both staff transferred from another center that is not operated by the same Limited Liability Company (LLC). The staff must be considered as new employees with new searches being conducted.

2 of 2 staff records reviewed did not include a SDS prior to employment at this facility. Both staff transferred from another center that is not operated by the same Limited Liability Company (LLC). The cots in the 2's rooms and the preschool room were not labeled with names for use by specific children.
Evidence:
At approximately 2:40PM the younger 2's room was observed to have 8 children resting on cots. There was not 12" of space between all occupied cots on the carpeted area.

3.Staff's hands were not washed between diaper changes for children.
Evidence:
*During the inspection in the Toddler room, LI observed multiple children receive a diaper change and not have their hands washed afterwards.
*The same multiple children were then seated in feeding chairs and given snack.
*LI observed staff change 3 child diapers without washing her hands between the diaper changes.
*In the Preschool room, children did not wash hands prior to eating snack. Children's hands were washed prior to lying down on cots for rest; however, all children did not sleep. 15 of the 17 children were observed by the LI awake and playing with toys, playing with shoes, or putting their hands on the floor.
Evidence:
In the Toddler room, LI observed a staff member using her hand to open the diaper disposable container. The container is a foot operated trash can; however, the can is placed on an elevated container where the foot operated pedal cannot be used.
Evidence:
LI observed a child in the Preschool room ask for more snack. The staff member replied there was no more snack available.

No inspection report is posted at the facility.
