Inspector Notes
An in person renewal study was conducted today.
All rooms are in ratio during the inspection.
Children were observed playing on the playground, washing hands, eating lunch, during diapering, being fed in the infant room, circle time and preparing for rest time.
4 staff and 4 child records were reviewed. Additional items reviewed: medications, emergency evacuation drills, allergy lists, allergy action plans, cot spacing, menu, emergency and first aid supplies.

The TB screening documentation on file is dated 7/22/21. Staff transferred from a different center that is not in the same LLC.
(Staff A Date of Hire: 01/23/23).
Evidence:
2 of 2 infant records reviewed. did not include documentation of 'Tummy Time" for either infant.
Evidence:
10 injury reports were reviewed.
7 of 10 did not include documentation of when the parent was notified of the injury. "At pick up" is not an acceptable time, as this time may vary.
Evidence:
1 of 4 child records reviewed did not contain acceptable documentation of having viewed a Proof of Birth document. Documentation on file was for a Social Security number. (Child 1)
