Inspector Notes
An unannounced monitoring inspection was conducted today. All rooms were in ratio during the inspection.
The 3s and 4s rooms were combined into the 3?s room and did not separate until 11:25AM.
Children were observed washing hands, eating lunch, transitioning from one room to another, playing musical instruments, playing with grocery items, and doing flashcards for animal sounds.
5 child records were reviewed. All were complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. LI was present today from 11:20AM -12:35PM.
Violations
6Based on observation, children under 10 years of age were not always within actual sight and sound supervision.
1. At approximately 11:25 AM the 3?s and 4?s combined classrooms were separating (leaving the 3?s room). Children were sent into the 4?s classroom without a teacher present in the area for approximately 1 minute. A staff member finally entered the room after the LI and a second staff person stated there was no staff in the room.
2. At approximately 11:35 child 1 was not supervised in the 4?s classroom. Staff were unaware that the child had searched through other children?s backpacks. Child 1 took a toy from Child 2?s backpack. When Child 2 went to put the toy back in their backpack, Child 1 began to cry, yell and have a tantrum. No staff member was aware of what the children were doing and unable to intervene as necessary.
Evidence:
Children were sitting on the carpet in the 4?s classroom for approximately 15 minutes, while 1 staff member prepared/set up for lunch, and a second staff corrected the attendance sheet to accurately reflect the classroom attendance. Children were hitting each other, rolling around on the carpet and each other with no staff interaction.
Evidence:
1. Child 1 was to leave the center to go to another program at 12:00PM. At 11:45 AM the child was instructed to put on their coat, and ?get ready to go?. No further instruction or assistance was given to the child.
2. Staff C told the child to get ready, took a toy from the child causing the child to cry and scream.
3. Staff C physically picked up the child to get them out the door of the facility.
Evidence:
During lunch, a child asked a staff member for a second of mango. The staff member replied, ?there was no more mango?.
Evidence:
During lunch in the 4?s classroom, no staff members were sitting with children during the meal. Staff were setting out rest cots and cleaning up.
Evidence:
1. 29 children were present in the combined classroom; however, the attendance sheet for the 3 and 4?s classrooms had 28 children present.
2. The Young Two?s room had a Toddler written in on the attendance sheet, then ?whited out? from the sheet. (Child 4)
3. The Toddler room had 6 children on the attendance sheet. There were 5 in the room. One child had been signed in twice, without ever being signed out to another room. (Child 4)
