Inspector Notes
An unannounced monitoring inspection was conducted today.
The playground was not in use due to construction of a new fence.
Children were observed; washing hands, during a diaper change, eating lunch, reading books while waiting for lunch, preparing for rest and resting quietly.

5Evidence:
1. Child #1 (23 months old) was moved to the 2?s-1 classroom. The applicable ratio was 1:5 (staff:child). There was 1 staff with 8 children present in the room.
2. Children #2 (2 years old) and # 3 (2 years old) were moved to the Preschool 3 room. The applicable ratio was 1:8 (staff:child). There were 2 staff with 17 children present in the room.
Evidence:
1. Child #5 was moved from the 2?s-1 to 2?s-2 room to maintain ratios. (dates)
2. Children #2 and 3 were moved from the 2?s-2 room to the PS-3 room to maintain ratios.(date s)
3. Child #1 was moved from the Toddler room to the 2?s-1 room to maintain ratios. (dates)
There were no written assessments were available for review for any of the children.
*Children were in their assigned classes.
Evidence:
Rest cots in the Toddler room were not identified for use by specific children with any identifying labels such as names or numbers.
Evidence:
1. Multiple rest cots in the Toddler room had less than 2 ?of space between the cots. Multiple cots were touching each other at the head and foot of cots next to each other. 2 cots were touching on the sides.
2. 4 cots in the 3?s room were touching at the foot of one cot and the head of the next one.
The center did not maintain an attendance sheet for each group of children that documented arrival and departure from the group.
Evidence: (DATE)
1. The attendance sheet in the 2?s -2 room reflected 2 children as departed from the room. (Children #3, 4)
2. Child #4 was marked as departed to another room but was not moved.
3. Child #2 had departed the 2?s-2 room; however, was marked as present in the 2?s-2 room.
Manager checks 3x per day.
