Inspector Notes
An unannounced inspection was initiated on 08/07/2024 and completed on 08/16/2024 in response to a complaint received by the licensing office on 7/30/2024 relating to allegations of supervision, ratio and diapering. This inspection included document review, tour of the facility, interviews, observations, and measurements as requested by the facility.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 30, 2024. A POC submitted after this date will not appear on the public website.
Violations
4Based staff statement and observation of video, a ratio of 1 staff to 8 children aged two years old was not maintained.
Evidence:
1. At approximately 10:45 on 7/26/24 11 children from a 2 years old room were on the playground with 2 staff.
2. Staff A left the playground to take 2 children to the bathroom, leaving Staff B with 9 children on the playground.
3. Staff A stated they left the playground with 2 children.
Based on staff statement and observation, there is no non-absorbent surface for diapering or changing for children under three years old.
Evidence:
1. There is no diaper changing table or surface designated for diapering/changing in the Discovery B room (2 years old)
2. Child #1's soiled diaper was changed on 7/26/24 while the child was standing up.
3. While changing the soiled diaper, the child's clothing became soiled with fecal matter resulting in the child leaving the center with soiled clothing.
4. Staff A stated that stand up diapering is done in the bathroom for Discovery B class.
Based on a review of an injury report, the information on the report did not accurately reflect the injury.
Evidence:
1. Staff A completed an injury report for Child #1 for an injury that occurred 7/26/24. Staff A was unaware of the circumstances of the injury and reported the child's hand had been scraped. The child actually had a mulch splinter embedded in their hand.
2. The accident report form does not include all required components. Staff present are not listed.
Evidence:
1. Child #1 received an injury at the center on July 26, 2024.
2. Parent informed the center at pick up on 7/26/24 they were going to the doctor's office for the injury. Parent followed up on 7/28/24 informing the center via email, that the child had been taken to the doctor on 7/26/24,
3. No report was made to the department's representative as of 8/7/24.
