During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/28/24 as a part of the licensure period. The Department issued a NOTICE OF INTENT TO PLACE LICENSEE ON PROBATION AND REQUIRE THE LICENSEE TO CONTACT PARENTS, GUARDIANS, OR OTHER RESPONSIBLE PERSONS REGARDING HEALTH AND SAFETY VIOLATIONS on February 2, 2024 as a result of high-risk violations. The on-site inspection began at 10:55 AM and ended at 1:05 PM.. The inspector reviewed compliance in the areas listed above. There were 49 children present and 11 staff. The inspector reviewed 3 children?s records and 2 staff records on-site. This inspection included: document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Each Center shall maintain a separate record for each child enrolled that includes a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the steps to be taken in the event of a suspected or confirmed allergic reaction.

Child #1's allergy action plan from the physician cannot be followed as a medication listed to administer does not have a completed authorization form on file.
Plan of Correction: The parent was contacted during the inspection to complete an authorization form for the medication.
