During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 8/11/2025, as a part of the licensure period. The on-site inspection began at 10:00amand ended at 12:35pm. The inspector reviewed compliance in the areas listed above. There were 75 children present and 12 staff. The inspector reviewed 7 children?s records and 5 staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care.

No documentation that Staff #1 has obtained required 16 hours of training annually 2024-2025. Staff #1 has been employed at the center for over a year.
Plan of Correction: Staff scheduled to participate in training by end October to obtain required hours on required topics.

Documentation shall be maintained of emergency evacuation, shelter-in-place, and lockdown drills that includes the number of staff participating.
