Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated and concluded on July20, 2021. There were 102 children present, ranging from ages 2 years old to 12 years old with 18 staff supervising. The inspector reviewed compliance in the areas, of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, the center failed to have an emergency evacuation and shelter in place map posted in a location conspicuous to staff and children.

Evidence:
Classroom Pre-K 1 did not have emergency evacuation and shelter in place procedures/maps posted.

Based on a review of staff record, 3 of 5 staff records reviewed did not have documentation that the center received completed fingerprint verification for new employees prior to the first date of employment.

Evidence:
Staff #1' start date was 3.26.2021 and has a fingerprint check dated 4.28.2021.
Staff #2's start date was 3.30.2021 and has a fingerprint check date of 4.6.2021.
Staff #3's start date was 6.16.2021 and has a fingerprint check date of6.21.2021.
Plan of Correction: All three staff members have current fingerprint checks in staff folders.
