Inspector Notes
Conducted an unannounced complaint investigation at 11:50am into allegations concerning the care of children. Observations made, staff interviewed and records reviewed and. Based on the evidence collected some concerns reported were determined to be valid while there was insufficient evidence to support other concerns reported and therefore determined to be not valid. Areas of non-compliance with standards and laws reviewed were found. Questions about this inspection may be directed to pamela.sneed@doe.virginia.gov

Violations
6Based on observations made, the findings from the most recent inspection were not posted on the licensed premises. Evidence: The findings from the complaint investigation report dated 9/27/23-10/6/23 were not posted on the licensed premises.
Based on observations made, the supervising staff did not ensure the care of 4 of the 21 children in the Preschool-1 classroom today. Evidence: 4 children were observed to have runny noses. The mucus from their noses was running from their nose to their mouths while eating lunch. At no time did the supervising staff attempt to wipe the children's noses.
Evidence:
1. At approximately 12:35pm, there were 3 staff, 1 staff trainee (not counted in the ratios) and 21 children present in the Preschool-1 classroom when Staff #10 exited the room.
2. When Staff #10 left the classroom the ratios became 2 staff to 21 children.
3. Children were having diapers changed and eating lunch at this time, while 1 of the 2 remaining staff was changing diapers in the diaper changing area the entire time.
Evidence:
1. Staff #4 was observed changing 6 children's diapers and assisting an additional child in the bathroom without washing her hands between each diapering/toileting. The staff wore and changed disposable gloves.
2. A child in the Preschool-1 class was observed having their diaper changed and when finished walked to a table in the classroom where they sat down and began eating lunch.
3. The child did not wash their hands and none of the staff prompted the child to do so.
Based on records reviewed, staff did not ensure that a written injury report was written to document facial scratches on 10/6/23.
Evidence:
1. A parent observed facial scratches on their child when they picked up the child on 10/6/23.
2. When the parent asked staff how the child got scratched, staff stated they did not know.
3. There was no documentation of a written injury report given to the parent on 10/6/23.
Evidence:
1. None of the 4 staff sat with the children during lunch in the Preschool-1 class.
2. None of the 2 staff sat with the children during lunch in the Pre-K class (today the Pre-K classes were combined into one class).
