Inspector Notes

An unannounced, on-site focused inspection was conducted on 1/28/25, in response to a self-reported incident. The on-site inspection began at 10:45amand ended at 12:18pm. The inspector reviewed compliance in the areas listed above. This inspection included document review and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 2/26/25. A POC submitted after this date will not appear on the public website.

When staff are supervising children, they shall always ensure their care and protection.

On 1/15/25, Child #3 fell during a dance activity after bumping into another child. There were 3 staff present at the time of the accident and all staff stated they did not inquire to see why the child was crying or what hurt and acknowledged they did not provide first aid to the child. Child #3 was picked up 19 minutes after the accident at which time staff provided the parent with an incomplete verbal accounting of the accident. The parent observed the child crying and not being able to put any weight on the wrist and called back to the center for additional details, as a written injury report was not provided to the parent at pickup. The child's injury required outside medical care.
