During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the program was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 9/9/25. The on-site inspection began at 9:50am and ended at 1:20pm. The inspector reviewed compliance in the areas listed above. There were 116 children present and 22 staff. The inspector reviewed 3 children?s records and 10 staff records while on-site. This inspection included document review, tour of the facility and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/16/25. A POC submitted after this date will not appear on the public website.

An out-of-state child abuse and neglect registry search and sex offender registry search shall be completed prior to employment for each staff person that has resided out of Virginia in the previous 5 years.

1. A sex offender registry search was not completed for Staff #15 who began employment in June 2025.

2. An out-of-state child abuse and neglect registry search and sex offender registry search were not completed for Staff #17 who began working in June 2025.
Plan of Correction: Completed all registry searches on noted staff.

Areas of the center, inside and outside, shall be maintained in a clean condition.

The floor in the stairwell, located adjacent to the outdoor playground, was dirty with grass, leaves, twigs/wood chips, trash and other outdoor debris. [Refer to Photo #1, #2 and #3]
Plan of Correction: Spoke with [name of county personnel] of Fairfax County cleaning crew to ensure that the area is kept clean. Forwarded photos from licensing.
Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

Two (2) children were observed entering the building stairwell, from the outdoor playground, where they remained for approximately 1 minute without the knowledge of the assigned staff to the school-age group who remained outside.
Plan of Correction: Will have staff meeting to review proper supervision of all children in our care.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include specific information, as listed below:

5 of 10 written injury reports did not include how the parent was notified, and 2 of 10 reports did not include the date and time the parent was notified.
Plan of Correction: Reviewed with staff proper procedure for completing injury reports.
