Inspector Notes

An unannounced monitoring inspection was conducted on 5/22/2024. There were 48 children, ages 2 years-5 years, 10 staff and 2 administrative staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. Medications were observed during the inspection. A total of 3 children?s records and 2 staff records were reviewed. The children were observed during active time on the playground, craft time and snack time. The inspector arrived for the inspection at 9:15am and departed the center at 11:00am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review and discussion with staff, the center failed to ensure that the separate record for each child enrolled contains all of the elements as required by the standards.

Evidence: The record for Child 3 did not contain the following information: documentation of viewing proof of the child?s identity and age
