Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 11/8/2024 and completed on 11/8/2024, as a part of the licensure period. The on-site inspection began at 10:30AM and ended at 12:34PM. The inspector reviewed compliance in the areas listed above. There were 49 children present and 11 staff. The inspector reviewed 3 children?s records on site on 11/8/2024 and 4 staff records on 11/8/2024. Medication was reviewed on site 11/8/2024.

This inspection included - document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, and observations (this would include observations of photos, recordings, etc.).

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Each staff member shall submit documentation of a negative tuberculosis screening within the last 30 calendar days of the date of employment. Staff #4, employed for 17 months, had a negative tuberculosis screening dated prior to the first 30 days of employment.
Plan of Correction: Provider will ensure staff have a tuberculosis screening within the first 30 days of employment.
