Inspector Notes

An unannounced complaint inspection was initiated on 01/24/2022 and completed on 01/27/2022 in response to an allegation of inadequate supervision and care of children.

Based on the information gathered which included staff interviews, a review of written records, a review of the facility and a review of center video this complaint is found to be not valid.

Two violations were found, that were not complaint related, in the area of children's records.

These violations were reviewed with the center director and are listed on the violation notice issued to the facility.

Standard 8VAC20-780-130-E
Based on record review and interview, the center failed to obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence:
1. Child 1, age 14 months, lacks documentation of current immunizations.
a. The most recent immunization record on file indicates the most recent immunizations were administered on 01/12/2021.
2. Administrative staff confirmed that an updated immunization had not been obtained for child 1.
Plan of Correction: Child 1 is no longer enrolled. All children?s files will be reviewed for compliance by 2/28/2022. Each month all children?s files that need updates will be flagged to ensure we collect updated records. Parents will be given 72 hours to provide updated documents or care will be suspended until file is complete. Management will review to ensure compliance.
Standard 8VAC20-780-140-A
Based on record review and interview, the center failed to ensure that each child shall have a physical examination by or under the direction of a physician before enrollment or within one month after attendance.

Evidence:
1. Child 1, enrollment date 02/01/2021, lacks documentation of a physical exam.
2. Administrative staff confirmed that a written physical exam was not on file for child 1.
Plan of Correction: Child 1 is no longer enrolled. Going forward all children will have a paperwork appointment before they start to ensure we have all proper paperwork. Management will inspect for compliance before child starts.
