Inspector Notes
An unannounced monitoring inspection was conducted on April 22, 2024 from 12:00pm to 2:35pm. At the time of the inspection there were 67 children in care and 11 staff members. Children were observed participating in various activities including handwashing, lunch and nap. Records were reviewed for five children and five staff members. Evacuation drills, emergency supplies and other required records and postings were reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed with the director during the exit interview.

5Based on record review, the center did not ensure that documentation of additional immunizations once every six months for children under the age of two years were in the record.
Evidence:
1. The most recent immunizations on file for child #2 (age 15 months), were administered on March 16, 2023, according to documentation on file.
2. Staff #1 (Director) confirmed that immunization record has not been updated.

1. The toddler room (age 16 months to two years old) had 10 children and 2 staff when reviewed by the inspector at approximately 2:50pm. The staff was using a 1:5 ratio but should have been using a 1:4 ratio because one child in care, Child #1, was 15 months.

1. Inspector observed a diapering cleaning process and the staff was observed wiping the sanitizing solution immediately after spraying it on the changing table.
a. "Sanitized" means treated in such a way to remove bacteria and viruses from inanimate surfaces through using a disinfectant solution (i.e., bleach solution or commercial chemical disinfectant) or physical agent (e.g., heat). The surface of the item is sprayed or dipped into the disinfectant solution and allowed to air dry on the surface for a minimum of two minutes or according to the disinfectant solution instructions.
2. Staff #1 (Director) confirmed that the staff wiped the sanitizing solution immediately after spraying it on the changing table.
Evidence:
1. The state inspection sticker for the bus expired in November 2023.
