Inspector Notes

An unannounced, on-site monitoring inspection was initiated on April 3, 2025 and completed on April 3, 2025. The on-site inspection began at 12:30pm and ended at 2:40pm. The inspector reviewed compliance in the areas listed above. There were 81 children present and 9 staff. The inspector reviewed 8 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (4/11/2025). A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations before the child can attend the center. The record for child #1 who was present at the center did not contain immunization records.
Plan of Correction: Director will ensure before start date that children have immunization record and physical, will update children tracking workbook and communicate with parents about upcoming expiration dates and suspend care when datelines are not met.
Standard 8VAC20-780-245-A
Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care. The record for staff #1 did not contain documentation of at least 16 hours of training for 2024.
Plan of Correction: Staff #1 has been assigned training hours with a completion date of 4/18/25. Director will monitor every staff training hours to be in compliance with 16 hour requirement. Director will assign training in a timely manner and will update staff tracking workbook accurately to ensure trainings are completed.
Standard 8VAC20-780-440-E
There shall be at least 12 inches of space between occupied cots, beds, and rest mats. Multiple occupied cots in the center had less than 12 inches of spacing in between them.
Plan of Correction: Director will retrain staff using the ruler method when placing cots to ensure there is 12 inches of space.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information: When applicable, work phone number and place of employment of each parent who has custody, name and phone number of the child's physician and the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
1. The record for child #1 did not have the work information for one parent and did not have the address listed for emergency contact #2.
2. The record for child #2, child #3, child #4 and child #5 did not have the work information for both parents.
3. The record for child #2 and child #5 did not have a second emergency contact listed.
Plan of Correction: Moving forward every child will have paperwork appointment prior to enrollment start date to ensure that all emergency contact and parent work information including phone number and addresses are listed, or N/A applied if a parent does not have employment.
