Inspector Notes

A monitoring inspection was conducted today between 12:00pm and 1:30pm regarding a self reported supervision incident. Statements were received and interviews were conducted. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-340-F
Based on staff interviews and statements, children under ten years of age were not always within actual sight and sound supervision of staff.
Evidence
Staff #1 was observed by staff #2 sleeping for approximately 3-6 minutes during the children's naptime. Approximately 12 or the 14 children were asleep.
Plan of Correction: Abra director will retrain all staff on sight and sound regulations issued from VA licensing. Abra director will also retrain all on staff Abra?s policies and procedures.
Standard 8VAC20-780-40-E
Based on interviews and statements from staff, the operational responsibilities of the licensee did not ensure the center's own policies and procedure were enforced.
Evidence
Staff #2 did not enforce the centers own policy and procedure. Center policy states staff members may not sleep in room where children are. Center policy states children must be supervised at all times. Staff #1 left children out of sight for approximately 3-6 minutes.
Plan of Correction: Abra director immediately terminated LaWanda Cole for violating licensing regulations and centers policy.
