Inspector Notes

An unannounced renewal inspection was conducted on December 5, 2022. There were 23 children present with 5 staff supervising. The physical plant, children's records, staff records, menu's, first aid kits, evacuation logs, attendance logs and Health Department certifications were reviewed. Based on a review of children's records, the center failed to obtain all required documents for each child enrolled.

Evidence:
- Child #2, #3 and #5's records do not contain documentation of the work phone number of each parent with custody of the child.
- Child # 2, #4 and #5's records do not contain documentation of persons, other than parents, who are authorized to pick up child.
