Inspector Notes

A monitoring inspection was conducted virtually with the director. Forty-one children were present with six staff providing supervision.
Two staff and two children's files were reviewed. The areas of the standards reviewed included administration, staff qualifications and training, physical plant, staffing and supervision, programs, special care provisions and emergencies and special services.
There was one violation notice issued.

Based on a review of two staff files and an interview with the director, it was determined that one staff did not have a repeat background check every five years.
Evidence:
Staff A's central registry finding was dated 2/12/17 and had not been repeated in more than 5 years.
Plan of Correction: We will send off for this background check within 10 days and forward Licensing the results upon receipt.
